Passport to Living Healthy

Welcome to the Passport to Living Healthy Program

More than 9,000 of our members have already joined us on this special health journey!

Your Passport to Living Healthy makes it easier than ever to follow up on recommended checkups, screenings, and immunizations.

The program includes:

  • Information on important preventive care screenings, tests, and immunizations, including when you should receive them.
  • Important questions to ask your primary care provider, from physical and mental health issues to how to avoid falls.
  • Specialized passports for managing chronic conditions, including asthma, diabetes, chronic obstructive pulmonary disease (COPD) and heart failure.

Plus, you'll receive free gifts as your passport is stamped!  You will get a free gift, if you complete one or more of eligible tests, exams, health checks or shots by the end of the year.  Take a look at the Gift Catalog link on the left.

The Living Healthy Program is free to all eligible members of Health Plan of Nevada and Sierra Health and Life. To join, please call (702) 933-1370 or toll-free at (877) 369-2199; TTY: 711.  We are open Monday through Friday, 8 a.m. to 5 p.m. PST.
