Are you eligible to join Sierra Spectrum?
To be eligible for Sierra Spectrum (PPO), you must:
- Be a Medicare beneficiary living within Washington County, Utah
- Be enrolled in Medicare Parts A and B
- Continue to pay Medicare Part B premiums and Part A premiums that apply
As specified by standards from the Centers for Medicare & Medicaid
Services (CMS), you will not normally be eligible to enroll in Sierra Spectrum
if you:
- Have end stage renal disease (ESRD) - that is, permanent kidney failure
that requires regular renal dialysis or a transplant to sustain life.
However, if you are already a member of our organization and have been since
your dialysis began, you may be eligible.
